FDA Launches Mobile-Friendly Safety Reporting Portal Tobacco Questionnaire

 

FDA’s Safety Reporting Portal (SRP) is a website that allows members of the public to report health or quality problems with tobacco products to FDA. In order to help users report health or tobacco product problems expeditiously and immediately after an adverse experience occurs, FDA has now launched a mobile-friendly version of the tobacco questionnaire.

To use the mobile-friendly version of the SRP tobacco questionnaire, please use the “Report as Guest” option. Note that logging into an existing SRP account from a smartphone or tablet will take users to the full desktop-version tobacco questionnaire.

Details about health or quality problems with tobacco products are important in helping FDA identify concerning trends, patterns and/or causes for reported incidents. Additionally, the mobile-friendly tobacco questionnaire requires users to submit at least one image (suggested to be the product, the label, and/or the injury), which can provide important details.

FDA encourages anyone who has had a reaction to, or was hurt by, a tobacco product – or knows someone who experienced such effects – to visit the SRP and provide as much information as possible. Reports may be submitted anonymously but including contact information allows FDA the option to find out more about the problem. FDA reviews all tobacco-related SRP reports to identify new or concerning trends.
